 Medicines Reconciliation Minimize

A workshop was organised at short notice in July 2007 to gather information on the services that have either been piloted or are currently provided in New Zealand hospitals. The information was to provide background to a project plan to submit to the Quality Improvement Committee (QIC). Six District Health Boards were represented at the meeting and a further two provided information on the services they currently provide by telephone.

Medicines reconciliation forms part of the Safe Medication Management project proposed by QIC that has been approved by the Minister of Health. Work on this is ongoing and pilots are in progress around New Zealand. National rollout to all DHBs is now in the planning stages and a preliminary workshop was held in July 2010.

Medicines reconciliation is being implemented in all DHBs during 2011. Future work will look at patient prioritisation and quality assurance.
